Each time you do this you can hire one new companion. Simply give away water or hack terminals in Megaton to adjust karma as needed. This way you can actually form a gang in the Wasteland and have as many people as you can afford follow you. It's like Fallout 1 and 2 where you can have 5-6 people at once.

A few last things to note: changing your karma level temporarily to get a specific companion is very easy to do. Also, Jericho, Clover, and RL-3 require a payment of 1000 caps (500 with high enough Barter) to be recruited, in addition to the karma requirement, and Charon requires a payment of 2000 caps or killing a specific NPC to be recruited
